Late Cancels
If you miss, change or cancel an appointment or class less than 24 hours notice you forfeiture that appointment or class.

Unlimited Membership

No-Show and/or Late Cancel Fee $20 is charged to the card on file.

Returned Item Fee

Payment returned for insufficient funds, etc.. $25.00

It is the client’s responsibility to book their classes. It is the client’s responsibility to cancel any booked classes that he/she cannot make. A member will lose a class credit from his or her membership package, for every occurrence, where he or she makes an appointment for a Forrest Pilates class and does not show up for it, or fails to cancel the scheduled class appointment at least 24 hours before the start of the class. If a member fails to cancel a booked class and has no class credits available for use, the credit card on file will be charged the member drop-in rate for the scheduled class. Failure to pay No-Show or Late Cancel fees may result in the suspension of your booking privileges.

Refunds

There are no refunds on products, services or gift certificates.

Membership Terms

Contract duration, automatic renewal, and agreement to pay recurring fees: contract is in force for one month from the contract start date, with automatic renewal for subsequent months scheduled one year at a time, unless cancelled by either the client or Forrest Pilates with a minimum notification of 14 days in advance of the next billing date. Client agrees to pay Forrest Pilates monthly recurring fees via client authorized automatic credit card or debit card payments in monthly increments. Payments by any other means than client’s credit card or bank account is not permitted. This autopay membership may be cancelled at any time. Memberships are for one year, no early termination fee, payment is month to month. To cancel, a request to cancel membership must be submitted in writing, email directly maria@forrestpilates.com​or hand deliver in person to Forrest Pilates, or send via certified mail to Forrest Pilates 936 NW 9^th CT Miami, FL 33136. Your written request to cancel your autopay membership must be received at least 30 days in advance of your next billing cycle.

• I agree to purchase the autopay membership indicated above for a one-time sign up fee of ​$30​plus the monthly fee, selected above, as an automatic charge to my credit card each month for a contract period of one month, with automatic renewal for subsequent months.
• As a recurring dues member, you are agreeing to pay for your membership on a recurring dues basis. Your recurring dues billing will begin on or before your 1^st session and will reoccur each month and will continue on the same day each month thereafter until you properly cancel.
• I hereby certify that I am the holder of the credit card, or an authorized signer thereof. You expressly authorize Forrest Pilates, or its third party billing services provider, to draft your account each month for any dues, fees, or other charges.
• Should any monthly payment become more than 5 days past due, I understand that I will be notified if my credit card fails to authorize for any reason, and that a $25 returned item late fee will be charged if I do not provide a valid credit card within 5 days of the original rejection date.
• I understand that my service will be deactivated if my account becomes more than 15 days late.
• I understand that from time to time, Forrest Pilates may, at its sole discretion, change any term and/or fee in this agreement, which will become effective upon (a) Forrest Pilates providing written notice of the proposed change; and (b) giving you 30 days after your receipt of such notice in which to cancel your agreement, penalty free, or renegotiate the change as to you. If, after a change is announced, you pay the next month’s invoice, you will be considered to have accepted the change.
• I understand that there are ​no rollovers ​or ​extensions​ on these monthly autopay memberships. ​All classes must be used within the month-long membership cycle that has been prepaid. ​Unused classes cannot be used the following month.
• To HOLD your AutoPay: a request must be submitted in writing, either hand delivered or via email to maria@forrestpilates.com​​at least 14 days in advance of your next billing cycle in order to be effective for the following month. Your billing date is based on your activation date listed above. You may put your account on hold for a minimum of a month and a maximum of 3 months. First membership hold is free and any subsequent holds will be charged a fee of ​$10/per hold ​to the credit card on file. Following the end of your requested hold period your monthly auto-renew will automatically be reactivated. No membership perks are available during a membership hold.
• You must promptly notify Forrest Pilates in writing of any changes to your billing information, address or telephone number. You expressly permit Forrest Pilates, or its authorized billing company, to obtain such updated information through payment card networks, card issuers or other third parties.
• Forrest Pilates reserves the right to terminate your membership for any reason not expressly prohibited by law. In the event of termination, we will notify you in person or send you a written notice of termination to your address on file, and refund you any unused prepaid dues.
• Purchasing Forrest Pilates Membership program and receipt of this contract, authorizes Forrest Pilates to charge your account at the discounted Monthly Membership rates outlined above for any further drop-ins you attend beyond your specified membership allotment each month. You understand that all memberships and discounts associated with membership are for the member (cannot be shared) and the service discounts are only for your membership.
